$1425Coordinating Activities, Rites and Ceremonies:
- Includes 24-hour staff availability, initial pre- or at-need arrangements and aftercare. This fee also covers the costs associated with taxes, insurance and licences.
- Professional consultation with a Class One Licenced Funeral Director, including, but not limited to, expert advice on options to meet your needs, and to accompany you throughout the provided services.
- Coordination of all activities, rites and ceremonies, before, during and after they have been provided. Coordination of services and supplies provided by us, as well as those provided by third-party suppliers, such as florists and newspapers (death notices).

$700Documentation – Permits, Forms etc…:
- Completion and filing of all documents (not including applicable fees associated with certain forms) necessary to carry out the services and supplies requested, including, but not limited to, death registration, burial permit, coroner’s certificate for cremation and/or documentation necessary to transfer the deceased out of the country.
- Provision of 8 Proof of Death Certificates issued by Bocchinfuso Funeral Home Inc.
- Also included:
- Cancellation of all government cards (SIN, Health Card, OAS Card, Pleasure Craft Operator Card, Outdoors Card [Fishing and Hunting Licence])
- Preparation and filing of the CPP Death Benefit and Survivors Benefit Applications
- Notifying the Canada Bereavement Registry to stop unwanted letter mail
- Notification sent to Canada Pension Plan and Old Age Security
- Use of a minimal register book for visitation and services, as required

(Within 25kms; additional fee applies beyond 25kms)
Staff services for the transportation of remains from one place to another. For example, from a hospital to the funeral home, or from the funeral home to the cemetery, mausoleum or crematorium. A charge applies for each transportation request.

$250Basic Preparation of Deceased Remains :
(Requires Facilities for Preparation/Embalming/ Shelter)
The preparation of the body for placement into the casket, including bathing, dressing and the setting of facial features for identification or viewing without embalming and checking remains for personal effects, jewellery and pacemaker. This charge applies whenever we handle deceased remains, and additional personal protective equipment when necessary.

Facilities used to prepare, embalm and shelter the body; this charge applies whenever the body is in our facility, for any purpose.

Mandatory minimum charge for the use of our general funeral home facilities, including parking lot, general areas, washrooms and other essential equipment required to operate as a licensed funeral home.
Vehicles

$250Lead/Administrative and Clergy Vehicle:
An automobile is used to transport staff for arrangements, services, obtain and deliver documents, transport clergy, and lead funeral processions.

An unmarked vehicle is used to transfer a deceased person from the place of death to a funeral establishment or to the crematorium.
